Liability-Release Waiver

By agreeing to this waiver, I represent that I understand the nature of Contact Football and Flag Football, and represent that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if, for any reason, I believe the event conditions to be unsafe for participation, I will immediately discontinue participating in the Activity. I acknowledge that I am fully responsible for determining for myself that all sporting and non-sporting events (and the activities there-in) hosted by Reflex Football, LLC, and/or its accompanying sponsors or partners, are safe for my attendance and/or my participation, as well as that of my family, friends, and anyone else that I may refer or invite to attend or participate.

I fully understand that this activity involves risks of serious bodily injury including permanent disability, concussion, loss of limb, paralysis, and even death, which may be caused by my own actions, or inactions, or by those of others participating in the event, or by the conditions in which the event takes place, or by the negligence of Reflex Football, LLC, and its accompanying sponsors/partners; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my attendance and participation in the activity.

I hereby forever release, discharge, and covenant not to sue Reflex Football, LLC, its respective administrators, directors, agents, officers, volunteers, facilities, and employees, other participants, any sponsors, any partners, any advertisers, and if applicable, any owners and lessors of premises on which the Activity takes place, (each considered one of the “RELEASES” herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence or the “releases” or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releases, I will indemnify, save, and hold harmless each of the releases from any loss, liability, damage, or cost which any may incur as the result of such claim. I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement and assurance of any nature and intend it be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

Media-Release Waiver

By agreeing to this waiver, I fully authorize Reflex Football, LLC, as well as all specified “Releases” listed below, to use the following personal information: (1) My picture – including photographic, motion picture, and electronic (video) images. (2) My voice – including sound and video recordings.

I hereby grant to Reflex Football, LLC, its subsidiaries, licensees, sponsors, partners, advertisers, successors, and assigns, the right to use, publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (video) form, sound and video recordings of my voice, and printed and electronic copy of the information described in sections (1) and (2) above in any and all media including, without limitation, cable and broadcast television and the Internet, and for exhibition, distribution, promotion, advertising, sale, press conferences, meetings, hearings, educational conferences and in brochures and other print media.

This permission extends to all languages, media, formats and markets now known or hereafter devised. This permission shall continue forever unless I revoke the permission in writing. I further grant Reflex Football all right, title, and interest that I may have in all finished pictures, negatives, reproductions, and copies of the original print, and further grant Reflex Football the right to give, sell, transfer, and exhibit the print in copies or facsimiles thereof, for marketing, communications, or advertising purposes, as it deems fit.

I hereby waive the right to receive any payment for signing this release and waive the right to receive any payment for Reflex Football’s use of any of the material described above for any of the purposes authorized by this release. I also waive any right to inspect or approve finished photographs, audio, video, multimedia, or advertising recordings and copy or printed matter or computer generated scanned image and other electronic media that may be used in conjunction therewith or to approve the eventual use that it might be applied. I acknowledge that I have read the foregoing and I fully understand the contents.

Communicable Diseases-Release Waiver

In consideration of being allowed to participate on behalf of Reflex Football, LLC, as well as all its respective administrators, directors, agents, officers, volunteers, facilities, and employees, other participants, any sponsors, any partners, any advertisers, and if applicable, any owners and lessors of premises on which the Activity takes place, athletic program and related events and activities including (but not limited to): football games, photo/video shoots, tryouts, draft events, award ceremonies, and other sporting or non-sporting events, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Reflex Football, LLC, as well as all specified “Releases” listed below, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
